Integrated Care for Dual Eligibles

People who have both Medicaid and Medicare, as well as a diagnosed disability, may experience fragmented care, which means that not all of the pieces of the service delivery system are working together for the good of the patient. For example, patients may be receiving prescriptions from more than one physician - often without the different doctors knowing about each other.  Fragmented care can lead to poor health outcomes.  Fragmented care is also costly.


There are few options in the current system of care.

Self Determination Policy & Practice Guideline

The Michigan Department of Community Health is extending the public comment period on the proposed revisions to the Self-Determination Policy until 5:00 p.m. on May 11, 2012.  Please send your comments to Cynthia Gilpin at gilpinc@michigan.gov. 

 

FOR IMMEDIATE RELEASE - May 18, 2012 - River Ranch Fresh Foods, LLC of Salinas, CA is initiating a voluntary recall of retail and foodservice bagged salads, because they have the potential of being contaminated with Listeria monocytogenes following routine random retail salad finished product tests conducted in the marketplace by the Food & Drug Administration.

Retail salad products under this recall were distributed nationwide under various sizes and packaged under the brand names of River Ranch, Farm Stand, Hy-Vee, Marketside, Shurfresh, and The Farmer’s Market.  Visit the FDA website for specific products and brand names.
